Our team will work with you using recovery tools to discover what matters to you, to develop skills and strategies to help you maintain good wellbeing, and to connect into meaningful community resources. Many of our team members have lived experience of recovery in mental health and bring that insight to support you on your journey.

Our support is offered on a one-to-one basis for a fixed period of time. Our team will work with you using recovery tools to discover what matters to you, to develop skills and strategies to help you maintain good wellbeing, and to connect into meaningful community resources. Many of team members have lived experience of recovery in mental health and bring their own insight to support you on your journey. Support is open to people leaving hospital and is offered on a short term basis. Our multi-disciplinary team includes Team Nurses, Recovery Practitioners and Workers, Peer Volunteers, Consultant Physician, Psychiatrist, Occupational Therapist, Clinical Psychologist, Physiotherapist and Social Worker. We will support your physical and mental health and build routine and structure in your life and you will become empowered to make informed choices about your daily living. We will support you to gain the skills you need to live as independently as possible and meaningfully to enable your return to your community or other suitable accommodation. We’ll work together with you to discover your strengths, and will support you to set and achieve your goals one step at a time.

We will support you to gain the skills you need to manage your tenancy and to live independently and meaningfully within your local community and support you to prepare to move on to more independent living in your own full tenancy.
